The treatment of infertility is one of the fastest advancing fields in medical science. Providing an accurate, complete diagnosis increases the chances of finding an appropriate treatment for infertility. Pre-registration

If you have not been a patient of The University Hospital or Community Clinics, you will need to pre-register at least 24 hours prior to your appointment, by calling (801) 297-4970.

Physician Referrals

As a reference lab and as part of our CAP certification, we are required to have a signed physician Requisition Order for all diagnostic tests, treatments, consultations, and procedures performed in the Andrology & IVF Laboratories.

Please ask your physician to fax the request to (801) 581-6127 at the time of the referral. The request needs to have your name (as the patient), the test or sperm preparation requested and the doctor’s signature. A semen analysis or other testing may be scheduled by calling (801) 581-3740.

Once the analysis is performed, the results are interpreted and mailed to the referring health care provider in seven to ten calendar days. The patient will need to contact his care provider for the results.

What to expect for your appointment

When scheduling an appointment, the patient will be advised to have two – five days of sexual abstinence prior to providing a semen sample. lan on approximately 20-45 minutes for the appointment.

1. Pre-register ((801) 297-4970) for your appointment 24 hours before your appointment
2. Bring your photo ID with you to your appointment
3. Bring or fax in advance a Requisition Order for the lab signed by your physician.
4. Sign-in at the front desk and wait to be called to the Andrology check-in desk. Payment is due at the time of service, a copy of your photo ID will be taken and demographics will be verified. A technician will review sample collection procedures, study patient consents and long-term storage agreements as needed.
5. Receive an insurance billing form receipt. (It is the patient’s responsibility to submit this form to the insurance company.)
6. Collect sample in a private collection room. (Sample Collection Instructions)

Please arrive a few minutes before the scheduled appointment, check in at the front desk and take a seat. Patients will be called to the Andrology office for service, which may include a short consultation, paperwork to process the sample, a review of semen collection instructions and payment (Fee Schedule & Policy ). Note All services must be paid for in full at the time they are provided. Upon completion of paperwork, with an opportunity for the patient to ask any questions or clarify information, the patient will be escorted to a private collection room. The technician will verify proper labeling of the container prior to collection and lock the door at his/her departure. The patient collects the sample and once complete, is free to leave, unless otherwise instructed.
