Referring a Patient for Ventricular Assist Device

We accept referrals via phone (801) 585-3188 or through our Mechanical Circulatory Support website.

 

In collaboration with the Heart Transplant Program, we will be glad to assist you in determining if a patient meets criteria for either cardiac transplant or VAD therapy. Our nurse coordinators will help facilitate the initial transplant or VAD evaluation. Our clinical team is also available for consultation 24 hours a day. Please call (801) 585-3188 to speak with one of our Transplant physicians or clinical coordinators. 

The following information will be requested at the time of referral:

  • Patient Demographics: Name, date of birth, gender, address, phone numbers, and insurance provider.
  • Patient Diagnosis- A brief medical history, all cardiac related diagnoses and procedures, and cardiac medications.
  • Referring Physician Information- Name, title, contact numbers, and office address.

A Successful Team of Multidisciplinary Professionals

 

The Cardiac Mechanical Support Program utilizes an interdisciplinary team approach which includes cardiac surgeons, cardiologists, registered nurses, pharmacists, cardiac rehabilitation specialists, dieticians, social workers, and financial coordinators who will thoroughly evaluate every patient prior to implantation. The patient’s wishes and expectations are the focus of the evaluation process. Every patient will establish personal goals they would like to achieve while living with a VAD. The Program Coordinators are available 24 hours a day to assist patients, their families, and providers with device related questions, education, and clinical support.
